Social Services Director

Licensed Social Worker - LSW

Grande Pointe Healthcare Community is seeking a Licensed Social Worker for the position of Social Services Director for our long term care facility. A 4 year degree in Social Work or related field and 3 - 5 years experience, including in a nursing home setting, are required.

Responsibilities of the Social Services Director include but are not limited to:

  • Provide planning, assessing, coordinating and implementation of services to enhance each resident’s social and psychosocial well being, including Discharge Planning.
  • Perform all duties involved in resident advocacy.
  • Participate in all resident care conferences.
  • Complete all social service related documentation.
  • Provide back up when necessary to the Admissions Department.

Job Requirements

  • A Degree in Social Work or related field, BA preferred.
  • Must have 2 or more years work experience in Social Services in a nursing home environment.
  • LSW required.
  • Must have previous supervisory/management experience.
  • Care Plan experience and MDS experience a plus.
  • Basic computer literacy
